HCPCS Level II doesn't have standalone vs add-on. Its more a piece together as many codes as needed to tell the story. For glasses you will have code for the lenses and a separate code for the Frames. If its just replacing a lens it will be a single lens code. same lenses new frame you would bill the frames and not the lenses.
