Billing 2 encounters in one day

My ortho wants to bill a patient for a follow-up healed fracture and then on a separate encounter bill for new problem for injection for peroneal tendinitis. He said because she has a different insurance for this problem. This is all in one visit. I have searched and cannot find a satisfying answer. Please help
